"She" is a hermaphrodite - somebody born with male and female genitalia. The term can also refer to plants. An earthworm is another example of a hermaphrodite.
Quite fascinating, if you were one you could give and take.  Apparently hermaphrodites take on the sex of the gender they're comfortable with.

no they are born naturally, and the chance of such born defects is very very low. even when born when both genetals, most of them have either one or both of them malfunction
